Pending means that there is a Payment that has been written but
not as yet settled.  If you pop up your History for that account/contract,
you will see that the Payment is sitting there with a Pending status,
you can hit Cancel any time to kill it.

Once cancelled, the Pending balance will go back to zero and the
main balance will go back to whatever it was.

Nothing's happening today, although yesterday we moved the
Issuer into a new rack, so it was down for a brief period.

Right, you can't type in the petname of the account
in that window - I guess that's a bug.  You either use
the dropdown or cut&paste a hash into the box.

>From a merchant perspective, you should definately do a
direct transfer - ask the customer to send you a zero
payment, and when you deposit that, add a name to it.
Basically, tell the customer you'll direct deposit to
an account of their choice, but you want to do a direct
deposit because you don't want the payment hanging around
for ever....

Of course, if you get the wrong zero payment, or they delete
the account, or lose their PC, then sorry!
