Overview

  1. This chapter describes the information needed to complete items 3 through 12 of Section IV, Form 4930, for input into the Criminal Investigation Management Information System (CIMIS).

Investigation Identification

  1. The following items contain the various codes and other information necessary to properly complete items 3 through 12 of Form 4930.

Item 3, Gender Code

  1. Item 3 "Gender Code" (P/S/SSI): This is a mandatory field for Primary Investigations, Subject Criminal Investigations, and Subject Seizure Investigations. Use one of the following codes listed below:

Item 4, Taxpayer Identification Number (TIN)

  1. Item 4 "TIN" (P/S/SSI): This is a mandatory and historical field for Primary Investigations, Subject Criminal Investigations and Subject Seizure Investigations. All TIN's (Taxpayer Identification Number) should be verified in IDRS using command codes INOLE or SCOLE prior to input of TIN into CIMIS. Enter the U.S. Social Security Number (SSN) or U.S. Employer Identification Number (EIN) for the subject of the investigation. If the SSN is unknown for gender code "M" or "F" or "O" , enter 000-00-0000. If the EIN is unknown for gender code "B" , enter 00-0000000. When the correct SSN or EIN is obtained, an update to this field will be necessary. Enter the SSN/EIN as follows: Enter the social security number using dashes where appropriate, i.e.,
    "987-65-4320." Enter the employer's identification number using a dash where appropriate, i.e., "32-1234567." If the wrong TIN has been entered into CIMIS notify CI Headquarters Hotline Staff.

Item 5 "Date of Birth"

  1. Item 5 "Date of Birth" (p/s): Enter for Primary and Subject Criminal Investigations only. Enter the birth date of the individual under investigation using the following format MMDDYY (i.e., "010561" , being January 5, 1961).

Item 6, Name

  1. Item 6 "Name" (G/P/S/SSI): This is a mandatory field for all types of investigations. Except for hyphens, do not use any punctuation or special characters in any of the name fields. Spaces or numbers may be used. For example, O'HARA would be entered as OHARA. General Investigations and business names must be entered in the Last Name field only. In sensitive situations, pseudonyms may be used to identify the investigation. This option should only be used upon approval of the Chief. The following are examples used in the Extended Name field:

Item 7, Address

  1. Item 7 "Address" (P/S/SSI): This is a mandatory field for Primary Investigations, Subject Criminal Investigations, and Subject Seizure Investigations. Enter the street address of the individual or entity the investigation involves, using abbreviations such as ST, AVE, DR, BLVD, etc., to conserve space. If the address is unknown, enter "UNK."

Item 8, City/Town

  1. Item 8 "City/Town" (P/S/SSI): This is a mandatory field for Primary Investigations, Subject Criminal Investigations, and Subject Seizure Investigations. Enter the city/town of the individual or entity the investigation involves. If the city/town is unknown, enter "UNK."

Item 9, State

  1. Item 9 "State" (P/S/SSI): This is a mandatory field for Primary Investigations, Subject Criminal Investigations, and Subject Seizure Investigations. Enter the appropriate code for the state or foreign country in which the individual or entity the investigation involves resides or is located from the list below. All states and foreign countries require four character designations.
    1. United States--State Abbreviations: The entry for any state and District of Columbia is "US." Listed below are the acceptable codes. Example: Illinois will be entered "ILUS." If the state is unknown, enter "XXUS."

  1. Item 10 "Zip Code" (P/S/SSI): This is a mandatory field for Primary Investigations, Subject Criminal Investigations, and Subject Seizure Investigations. Enter the appropriate zip code. If the exact U.S. zip code is unknown, enter "00000-0000." If a foreign country code is entered in Item 9 "State" , enter "99999-9999" in this item.

Item 11, Alias/DBA/Partnership

  1. Item 11 "Alias/DBA/Partnership" (s): Enter for Subject Criminal Investigations the appropriate one digit code from below to designate the entry of an alias, DBA (doing business as), or partnership name, followed by the name (i.e., enter "1" , followed by "BIG DADDY" to enter an alias, or "3" followed by "ABC Supplies" , to enter a partnership name). Except for hyphens, do not use any punctuation or special characters in this field. Spaces or numbers may be used. For example, O'HARA would be entered OHARA. The system will maintain a record of all entries in this multiple field.
